Please read the information below to better understand the advantages and limitations of a telehealth appointment.

By participating in a telemedicine appointment with Macungie Animal Hospital, I understand that

    1. Telemedicine technology will be used to connect me with a veterinarian. The consult may be conducted by videoconferencing, video images, still images, or by telephone conference. I understand that this appointment will not be the same as a direct patient/veterinarian visit due to the fact that the veterinarian will not be in the same room as my pet.
    2. In participating in a telemedicine visit for my pet, I realize that some aspects of my pet’s physical condition may remain undetermined. The veterinarian will visualize my pet’s current status but is unable to provide a comprehensive examination due to the limitations of telemedicine. I understand that if the veterinarian recommends further testing or medications, it will be my responsibility to visit Macungie Animal Hospital or another facility to fulfil those recommendations.
    3. There are potential risks to this technology, including interruptions, unauthorized access and technical difficulties. I understand that my veterinarian or I can discontinue the telehealth appointment if it is felt that the videoconferencing connections are not adequate for the situation. I understand that I can discontinue the telehealth appointment at any time.
    4. In an emergency situation, I may be directed to a veterinary emergency facility.
